Below is a list of required documents and information that is required to start the process. Please provide all necessary documents and complete the application form in it’s entirety to avoid any delays in processing.

  • W2s – last 2 years
  • Pay stubs – last 30 days’ income
  • Bank statements – last 2 months, ALL pages
  • Most recent mortgage statementMost recent Homeowner’s Insurance
  • Declaration Page
  • If you are self-employed:
  • Last 2 years’ Personal AND Business tax returns with ALL SCHEDULES
  • If you are a Non-Citizen
    • Front and back copy of Green Card OR
    • Current, unexpired copy of I-797 including I-94 or applicable Work Visa documentation
  • If you own any additional properties (provide for each property):
    • Most recent mortgage statement
    • Most recent Homeowner’s Insurance Declarations Page
    • Most recent Tax bill
    • Most recent lease agreement (if renting out)
    • Most recent HOA bill (if applicable)

NOTE: there is extra space on the last page of this application to include any additional employment information you may have to provide us with a full 2-year history.
